Most rejected leads at qualify() are rejected for the same handful of reasons. The fix is almost always the lead list, not the prompt.
Common rejection reasons

If a single reason accounts for more than half the rejects on a list, the list is upstream of the problem. Re-source. Do not lower the threshold.
